Preparing Multiple Choice Exams To Be Graded

(USE NUMBER 2 PENCIL ONLY)

1. Fill in green Header Sheet

2. Fill in gray Answer Key and Bubble in Sheet Type A to identify the key (gray paper size 8.5"x11", Form no.:NCS MP90051, NCS Pearson MM90051-2 or Scantron form No.0-101607-TAMU)

3. Fill in orange Form Sheet , Weight Sheet,Multiple Response Sheet as needed.

4. Arrange Students'Answer Sheets in the same direction (Student Answer sheets will be the same form number as mentioned in No.2)

5. Place the above sheets (No.1-4) together and put them in an envelope or a box for scanning and scoring
