When the Multiple Form test requires a method of item ordering different from the choices on the green Header Sheet, you should use a Form Sheet. Converting from Form A to another Form can be done by matching items, e.g. if item No.1 on Form A is the same as item No.4 on Form B, bubble in as shown. Continue corresponding answer. Indicate the item and form by bubbling in both 1-40 SHEET1, 40-80 SHEET2, 81-120 SHEET3, 121-150 SHEET4 and FORM A, B, C or D on this sheet. Skipping bubbles can be done only if the sequence of numbers increases by one.
